Do I need a referral letter to see a doctor in Japan?
Not for a clinic. But if you go to a large hospital (200+ beds) without a referral, many add a selection fee of about ¥7,700 per department. Start at a local clinic — they'll refer you if needed.
Which department should I go to for my symptom?
If you're unsure, start at an internal medicine (内科) clinic — it covers most everyday problems and can point you to a specialist. For severe or sudden symptoms, call 119.
How much does it cost? Do I need insurance?
With Japanese public insurance you pay 30% (a clinic visit is often ¥1,000–¥3,000). Without insurance you pay 100% upfront, and large/university hospitals may charge 2–3× the standard rate. Travel insurance is strongly recommended for visitors.
How do I find a clinic where English is really spoken?
'English OK' signs can be unreliable. Call ahead to confirm, check recent reviews from foreign patients, and bring a translation app — or ask us on LINE/WhatsApp and we'll help.

What number do I call in an emergency in Japan?
Call 119 for an ambulance in a life-threatening emergency. If you're unsure how serious it is, call #7119 for free advice first. For children, many areas have #8000.
Can I get a second opinion?
Yes. You can request your own medical records and take them to a hospital's second-opinion outpatient (セカンドオピニオン外来). A referral can also be written by a different doctor after reviewing your records.
I caught a cold — where do I go?
For a common cold, a regular internal medicine (内科 / naika) clinic is enough. You usually don't need ENT or a throat camera/scope.
How do I see a skin doctor (dermatologist), or get a rash checked?
Go to a dermatology clinic (皮膚科 / hifuka) — the 'skin doctor'. For a rash, acne, or a skin problem you don't need a referral; start at a local 皮膚科.
