A hospital bill in Japan has a few parts. Knowing them prevents surprises.

The main parts

• Base hospitalization fee (入院基本料) — covered by insurance at your usual rate.
• Meal fee (食事療養費) — a fixed daily amount for meals.
• Private-room fee (差額ベッド代) — an optional extra for a private/upgraded room. This is the one that surprises people.

About the private-room fee

You pay it only if you requested a private or upgraded room. A standard shared room has no extra fee. If the hospital places you in a private room for their own reasons (e.g., no shared bed was available) or out of medical necessity, you should not be charged — even if you end up in a private room. If this fee appears but you never asked for a private room, question it.

For insured patients

Ask about a 限度額適用認定証 (eligibility certificate) so high bills are capped up front via the 高額療養費 system.
